The Partnership for Chronic Disease Management in partnership with the Oregon Cascades West Council of Governments, is offering Living Well with Chronic Conditions workshops in Newport, Waldport and Siletz this fall. The Living Well with Chronic Conditions workshop is a chronic disease self-management program created by Stanford University, School of Medicine. Individuals who have attended the workshop, report less doctor visits, less trips to the emergency room and less over-night stays in the hospital. Most participants also report an over-all improved outlook on living a productive life with a chronic condition.
The workshop in Waldport is scheduled for Monday evenings, 4-6:30 pm, beginning on August 19 at the Our Savior’s Lutheran Church in Waldport. The workshop in Newport is scheduled to begin on September 11, Wednesday afternoons from 1:30-4:00 at Oceanview Senior Living. We also have a workshop series scheduled in Siletz at the Siletz Tribal Community Clinic later in the fall. We provide healthy snacks and water at each session. All participants will receive a copy of the companion book.
If you suffer from a chronic condition and would like help and support to learn how to live healthier and happier – this workshop is for you! The workshop is a six-session series, two and a half hours each week, for six weeks. We cover between four and six topics each week. The topics covered include: healthy eating, communication, better breathing, relaxation, medication, exercise, goals, problem solving and action planning, and many other topics. Participants self-report having numerous chronic conditions, including: diabetes, depression, heart disease, arthritis, asthma, multiple sclerosis, bronchitis and many others.
