VACCINATION CHOICES
First, some things to know before you get your vaccine
- COVID-19 VACCINES ARE ALWAYS FREE.
- You do NOT need to provide a social security number or ID.
- You do NOT have to pay for any administrative fees. If you ever get a bill for a COVID-19 vaccine, call the number on the bill and tell them you do not have to pay for any fees for the COVID-19 vaccine.
- Bring this with you to your appointment: Screening and Consent Form or Consent for Ages 14 and younger
- Eligible minors age 15 and older may receive the PFIZER vaccine without consent from their parents or guardian. The vaccines are free and health insurance is not required. MINORS’ RIGHTS TO HEALTHCARE Click here for information
- Need a 2nd dose? Attend one of the clinics listed below or check out these options here
- Need a 3rd dose (additional dose)? This is approved only for those who are immunocompromised and received the Moderna or Pfizer vaccine. See details on who is eligible here.
- Need a Booster? Pfizer is the only vaccine with a booster approved. Those age 65 and older and people who were in Phase 1A are eligible. See the full list of eligible people here.

Pfizer and Moderna vaccines will be administered at drop-in clinics, depending on location and availability. The second dose will be scheduled before you leave.

Vaccination and Testing Bus – in Lincoln County October 24 – November 5, 2021

Clinics at the mobile vaccination bus are open from 12pm to 7pm every day during the visit to Lincoln County. The locations are at

- Waldport: October 26 – 27 at the Waldport Community Center
- Toledo: October 28 – 30 at the Toledo Library
- Lincoln City: October 31 – November 5 at Chinook Winds Casino Resort

Transportation
If you need a ride to a testing site or vaccination clinic, here are some options:

How to replace your vaccination card
Lost your vaccination card? You can use your record of vaccination as proof.
If you have lost or damaged your vaccination card, you can use your record of vaccination as proof. Here’s how to get your record (replacement CDC cards are not available).
- Check with your health care provider, or their website, for your immunization record.
- Follow the instructions on the OHA Immunization Resources page. You will download the appropriate form and can email, fax or mail the completed form to the ALERT IIS. It may take 5 days to get your record.
- Call 211 for help in languages other than English.
